Our care managers frequently visit clients to spot check the aides and oversee the care.
We design and implement a care plan for the caregiver, family and aides.
We can schedule doctor and other medical visits, plus we coordinate transportation and provide aides to escort the client if needed.
Clerical and financial services
We can document and follow through with medication ordering and management.
We will assist and if need be refer you to the right resources in the complex process of applying for Medicaid, Community Medicaid, EPIC, Access-a-Ride, medical insurance and other paperwork so it gets done right.
We can arrange for in-home visits by medical personnel, x-ray and lab work.
Our care managers will respond during an emergency 24/7 to the hospital ER to stay with your loved one. We can also arrange care for and provide your loved one with overnight care for the duration of their stay and then help arrange a safe discharge.
We can also help prepare living wills, healthcare proxies and DNR documentation
